Writing as one who used to be part of a Engineering Consultancy, I can confirm that we all attended a five-day training session on how to avoid even mentioning the word ‘blame’, never mind accepting the tiniest portion of that dreaded term. Special emphasis was placed, during the extensive training periods, on the ability to point at everyone else, both associated with the project; or not. A special section of each tender document, which of course was never released to contractors, stated categorically that it was definitely someone’s fault, and never, ever, ours; but if a Tender was successful, the Contractor was deemed to be at fault from day One.
